1.
Constant communication with providers, patient's insurance and patient.
2.
Answering high volume of calls.
3.
Maintain ongoing tracking and appropriate documentation on referrals to ensure patient health is up-to-date.
4.
Assemble information concerning patient's clinical background and referral needs.
Per referral guidelines, provide appropriate clinical information to specialist.
5.
Contact review organizations and insurance companies to ensure pre-authorization requirements are met.
Present necessary medical information such as history, diagnosis and prognosis.
Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.
6.
Review details and expectations about the referral with patients.
7.
Be the system navigator and point of contact for patients and families, on patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion.
8.
Ensure that referrals are addressed in a timely manner.
9.
Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.
Qualifications:
1.
High school diploma, preferred if it's combined with medical assisting certificate 2.
Strong customer service focus.
3.
Attention to detail.
4.
Effective verbal and written communication skills in English.
Bi-lingual also a plus.
5.
Teamwork oriented, reliable and punctual.
6.
Maintaining patient's privacy and dignity.
7.
Organized and able to manage competing priorities.
8.
Good judgment.
9.
Resourcefulness in problem solving.
10.
Able to take and follow through with delegated tasks and accountability.
11.
Knowledge in EHR-Epic is preferred but not required.
Prior experience in medical office is a plus.
